Responsibilities & Context:
∎ Conducting screening and assessments for the project participant and implementing the Rehabilitation interventions.
∎ Developing comprehensive rehabilitation treatment plans for individuals requiring follow-up rehab services, ensuring a holistic approach to healthcare.
∎ Identifying specific needs for Assistive Devices, Home Modifications, and making necessary referrals for additional medical and rehabilitation services.
∎ Evaluating and selecting appropriate assistive devices tailored to the unique needs of each client, enhancing their mobility and overall well-being.
∎ Ensure partners have a clear understanding of the Disability Inclusion Work Plan and collaborate with them to implement its principles effectively.
∎ Providing disability inclusion capacity development support to all BHA agencies. Develop and execute training programs to empower staff within partner agencies.
∎ Provide technical support to partner agencies by promptly responding to requests for disability inclusion input and guidance on integrating disability inclusion into critical tools and documents.
∎ Participate in joint field visits alongside partner agencies. Provide on-the-job support and conduct monitoring activities to ensure the successful integration of disability inclusion practices.
∎ Develop and deliver training programs and workshops on standards, identification disability awareness, inclusion, and etiquette for staff, volunteers, and community members.
∎ Conduct accessibility assessments of different facilities to identify and address barriers, Provide recommendations for modifications.
∎ Create and distribute resources such as guides, toolkits, and educational materials on disability inclusion. Develop accessible IEC materials.
∎ Offer technical support to organizations in making their programs, services, and events accessible.
∎ Adhere to CDD's Children and Adults at Risk Safeguarding policy, ensuring the safety and well-being of all project participants.
∎ Promote a safe and secure environment for all participants and stakeholders of the project.
∎ Report any safeguarding concerns promptly and follow the designated reporting procedures.

∎ Duties and Responsibilities:
