Additional Requirements:

Minimum of 3 years in a similar role, preferably in emergency or humanitarian settings.

Language

• Mandatory fluency in Bangla, with additional proficiency in English and Chittagonian dialect considered an advantage.

• Excellent knowledge of clinical protocols, quality standards, and emergency response.

• Strong leadership and team management skills.

Job Purpose of Referral Facilitator:

The Referral Facilitator - NCD Program will support the clinical and health teams in the Rohingya camp by ensuring timely and efficient referral of individuals with non-communicable diseases to appropriate health facilities in Project for promotion of primary and secondary prevention of non-communicable diseases and strengthening its follow-up system in Forcibility Displaced Myanmar Nationals in Cox’s Bazar, Bangladesh. The role focuses on coordinating between CHWs, health facilities, and patients to streamline referral pathways, improve follow-up care, maintain accurate documentation, and promote access to continuous, patient-centered NCD services.

Job Responsibilities:

  • Active engagement with referral and management of transportation for the patients to higher facilities as per required.
  • Maintain referral slip and register.
  • Establish coordination and good linkage with Govt. Hospitals/Field Hospitals and other higher Health Facilities as per the referral pathway.
  • Maintain updated lists of referred and follow-up patients and provide necessary support throughout the referral process.
  • Coordinate closely with clinic staff to ensure regular follow-up and appropriate care of beneficiaries with NCDs.
  • Controlling the referral system, considering the Referral Pathway given by Health Sector.
  • Follow-up the referred patients.
  • Ensure the need-based support of the patients.
  • Maintain functional relations with and accountability to the in-charge of the respective Health Facilities.
  • Engage in other emergency activities as and when required.
  • Prepare the Monthly Report and submit within the prescribed date.
  • Prepare and submission of case study and success story to respective office regularly.
  • Liaise with government authorities, relevant stakeholders, and the project office to support effective programmatic coordination, administrative processes, and information sharing.
  • Quality, Ethics, and Compliance
  • Maintain patient confidentiality and follow organizational Code of Conduct, PSEA, and workplace safety policies.
  • Participate in regular team meetings for information sharing, coordination, and service improvement.
  • Attend trainings, capacity-building activities, and organizational programs as assigned.
  • Perform any other duties assigned by the authority relevant to Health Post operations.

    • Gross Salary: BDT 50,000 per month
