DMF (Diploma in Medical Faculty) or equivalent recognized qualification.·
Valid BMDC registration.
Experience and other requirements:
Minimum 2 years of relevant experience in health programs, PHC, community health, NGO, or development organizations.
Good knowledge of the Bangladesh health sector, primary healthcare system, and community-based health services.
Sound technical knowledge and practical skills in PHC service delivery.
Good skills in clinical service delivery, community mobilization, communication, reporting, documentation, and online data management.
Good interpersonal, teamwork, coordination, and problem-solving skills.
Ability to work effectively at both PHC and community levels.
Experience in coordination with government health facilities will be an advantage.
Name of Project: SAJIDA Primary Healthcare Center
The role is responsible for ensuring effective implementation and achievement of SAJIDA Primary Healthcare Center (PHC) objectives through quality healthcare service delivery, community outreach, patient care, proper documentation, reporting, and coordination with internal and external stakeholders.
A) Major Responsibilities
Provide quality Primary Healthcare (PHC), ANC and PNC services at the PHC.
Screen, identify, counsel, manage, and refer NCD and lifestyle-related cases as appropriate.
Plan and conduct satellite clinics, community outreach, health screening, and eye camps at PHC and community locations.
Conduct growth monitoring and nutritional assessment (GMP) for under-five children.
Provide basic mental health support and arrange teleconsultation/referral for complicated cases.
Conduct health education and awareness sessions for adolescents, women, and other community members.
Ensure appropriate referral linkages with SAJIDA Hospital, government hospitals, and other relevant health facilities.
Manage PHC-level medicine, FP commodities, hygiene products, and other logistics, including requisition, distribution, stock control, and documentation.
Ensure accurate recording of patient history, diagnosis, clinical advice, and services through the SF App and relevant registers.
Ensure proper collection and deposit of service-related cash in accordance with organizational policies.
Prepare and submit daily, weekly, monthly, quarterly, and annual reports with timely online data entry.
Monitor targets versus achievements, identify gaps, and take appropriate improvement actions.
Ensure compliance with SAJIDA safeguarding policies, standards, and procedures in all activities.
B) Minor Responsibilities
Maintain required patient, ANC/PNC, family planning, nutrition, eligible couple, stock, and other program registers.
Prepare monthly action plans and support their effective implementation.
Prepare and submit monthly success stories with photographs and signed consent forms.
Support special medical camps, cancer screening camps, and other health initiatives.
Organize and observe relevant national and international health days.
Maintain effective coordination and rapport with MF, government/local government, NGOs, community leaders, and other stakeholders.
Attend meetings, trainings, workshops, and other organizational activities as required.
Perform any other duties assigned by SAJIDA Foundation management.
