Responsibilities & Context:
∎ Facilitate to improve NCD services at Community Clinic (CC) through capacity building of CHCPs, HAs and FWAs
∎ Support improving the referral and back-to-back referral systems among CCs-Upazila Health Complex (UHC)s.
∎ Develop capacity of CGs and CSGs for NCD prevention and control at community level.
∎ Facilitate to strengthening linkages between CCs and local governments for coordination among key stakeholders, ensuring availability of health care services, enhancing accountability of health providers and local resource mobilization.
∎ Facilitate to strengthening the monitoring and supportive supervision to CC/CG/CSG by Core Team members at the Upazila levels using the monitoring checklists;
∎ Ensure facilitation support to develop annual plan of CG and CSG in regards with NCD related activities.
∎ Facilitate to share CC performance shrining by the Core Team Focal person at upazila health coordination meeting.

∎ The Project for Strengthening Health Care Systems for Preventing Non-Communicable Diseases known as SHASTO2, implemented by the Ministry of Health and Family Welfare and supported by JICA, is aimed at improving quality of essential NCD prevention and care services in rural areas in Bangladesh.
∎ With the financial and technical support of SHASTO 2, Diabetic Association of Bangladesh (BADAS) and SHED Foundation is implementing community-based intervention to strengthening the monitoring and supervision system of CC/CG/CSG through the functioning of the Core Team to promote NCD prevention and control at the community level.
∎ Job Responsibilities:
