Job Context:

Active in more than 45 countries, Tdh works with local and international partners to develop and implement field projects which improve the daily lives of over 3 million children and their relatives, in the domains of health, protection and emergency relief.
∎ Following the massive displacement of around 900,000 Rohingya people into Bangladesh, since the end of August 2017, Tdh is implementing an emergency response program to provide relief to the Rohingya refugees and host communities, notably on Child protection, WASH and Health.
∎ PHC is a health facility engaged in the provision of Primary Health Care to patients. Some of them require a higher level of medical treatment and thus have to be referred to other facilities for further attention. The referrals are made with ambulance and paramedical staff. Proper documents (referral forms) have to be issued and arrangements are done before the referral. The position holder will be responsible for the preparation of the referral process and help the medical staff and patients for this to be as smooth as possible.
Job Responsibilities:
∎ Maintain ongoing tracking and appropriate documentation on referrals to promote team awareness and ensure patient safety. This tracking may use an IT database.
∎ Ensure complete and accurate registration, including patient demographic and current insurance information.
∎ Assemble information concerning the patient's clinical background and referral needs. Per referral guidelines, provide appropriate clinical information to specialists.
∎ Close Contact with referral hospital for treatment and investigation cost for treatment of the patient.
∎ Present necessary medical information such as history, diagnosis, and prognosis.
∎ Provide specific medical information to financial services to maximize reimbursement to the hospital and physicians.
∎ Review details and expectations about the referral with patients.
∎ Assist patients in problem-solving potential issues related to the health care system, financial or social barriers (e.g. request interpreters as appropriate, transportation services or prescription assistance).
∎ Be the system navigator and point of contact for patients and families, with patients and families having direct access for asking questions and raising concerns. May assume an advocate role on the patient's behalf with the carrier to ensure approval of the necessary supplies/services for the patient in a timely fashion.
∎ Identify and utilize cultural and community resources. Establish and maintain relationships with identified service providers.
∎ Ensure that referrals are addressed in a timely manner.
∎ Remind patients of scheduled appointments via mail or phone.
∎ Ensure that patient's primary care chart is up to date with information on specialist consults, hospitalizations, ER visits, and community organizations related to their health.
∎ Maintain voucher for referral expenditure & give the voucher to the assigned person.
∎ Maintain money for the referral of patients.

Additional Requirements:
∎ Both males and females are allowed to apply
∎ Experience: Essential working experience of at least three years in similar activities related jobs. Extra courses or training will be an asset.
∎ Language: English and Bengali essential and Rohingya would be an asset.
