Job Responsibilities:
∎ Identify beneficiary/targets by conducting survey/household visits and update on quarterly basis, develop WARD action plan and implement to achieve results.
∎ Maintain linkage for service delivery at slum and health facility level with service providers of CODEC and other partners to ensure convergence and synergy.
∎ Support City Corporation officials/service providers for delivering nutrition services at health facilities five days in a week.
∎ Ensure nutrition counseling including IYCF at health facilities and at slum level.
∎ Ensure GMP and screening of targeted children for acute malnutrition and referral and follow up at slum level for SAM children, MNP supplementation and IFA supplementation for PLW and Adolescent girls at health facilities and at targeted slums.
∎ Support to organize adolescent groups for training, ensuring slum level adolescent nutrition counseling by peer leaders and service delivery (IFA supplementation and half yearly deworming etc) in targeted slums.
∎ Ensure referral linkage between slums and health facilities for nutrition services; Ensure availability, installation and use of supplies and logistics and IEC materials in health facilities and in the slums.
∎ Ensure registers (for PLW, U5 children, GMP, MNP supplementation, adolescent counseling etc) are in place, used for record keeping the services provided and generating reports etc.
∎ Visit households of PLW, U% children, SAM children, disables on regular basis in the slums provide necessary follow up and promotional supports and coordinate activities at the local level and maintain cooperation with community people and other partners and sectors of CODEC.
∎ Attend monthly trainings and be able to accurately replicate trainings received, sharing correct information and demonstrating skills learned.
∎ Support City Corporation and MoHFW and other partners for implementation and monitoring of national events like Vitamin-A campaign, National Nutrition Week, World Breast Feeding Week etc.
∎ Attend training and coordination meetings at the ward counselor office; provide verbal and written reports of activities completed.
∎ Prepare monthly report and submit to UNF and health facility manager/Zonal Medical Officer of City Corporation.
∎ Any other responsibility may be assigned by Urban Nutrition Facilitator and Program Coordinator as and when required.
