Responsibilities & Context:
∎ Under the guidance of the Nutrition supervisor- Nutritionist, the Nurse will be responsible for-
∎ Responsibility 1: Medical and Anthropometric Assessments and Referrals
∎ Conduct comprehensive assessments of mother/caregiver-infant pairs, focusing on:
∎ Medical evaluation of the infant’s health.
∎ Detailed anthropometric measurements, including weight, length, and mid-upper arm circumference (MUAC).
∎ Regularly assess infants for danger signs or acute health issues, utilizing IMCI guidelines, and perform triage to ensure prompt intervention.
∎ Perform medical monitoring tasks such as recording vital signs, conducting physical examinations, and reviewing medical history, identifying any indications of illness, such as fever, respiratory distress, dehydration, or nutritional oedema.
∎ Refer infants needing urgent care to the inpatient corner for immediate stabilization, including those with danger signs, severe malnutrition, or acute medical complications including signs of dehydration, malnutrition-related edema, or other critical conditions.
∎ Counsel caregivers on the benefits of regular follow-up visits, explaining the importance of consistent growth monitoring and early intervention.
∎ Monitor outpatient cases to ensure continued nutrition and health stability, supporting transition to the outpatient corner if suitable for ongoing care
∎ Responsibility 2: Monitoring and Supporting Feeding Practices through Effective Referral
∎ Conduct detailed assessments of feeding practices, including breastfeeding attachment, positioning, latch, and suckling efficacy.
∎ Identify feeding difficulties such as poor milk intake, maternal concerns about milk insufficiency, and any feeding aversion.
∎ Assess regarding the exclusive breastfeeding and/or any deviation from it.
∎ Refer to appropriate IYCF corners in case of any breastfeeding related challenges to support through counselling, cup feeding, milk expression etc.
∎ Refer to inpatient unit if required for higher level breastfeeding support like re-lactation, supplementary suckling technique etc.
∎ Closely monitor infants who are non-breastfed, ensuring effective referral to the designated points with permission to assess and provide guidance on formula preparation if required as a last resort.
∎ Responsibility 3: Psychosocial Support Coordination
∎ Conduct preliminary assessment on the symptoms of maternal depression, anxiety, or other mental health concerns.
∎ Conduct preliminary assessment on difficulties related to social support or environmental stressors.
∎ Refer mother-infant pairs with psychosocial needs to the Psychosocial Support (PSS) corner for further investigation and providing mental health counselling or support services.
∎ Collaborate with PSS staff to ensure comprehensive care, addressing both the physical and mental well-being of caregivers.
∎ Responsibility 4: Management of Pregnant and Breastfeeding Women (PBW)
∎ Accurately record anthropometric measurements and essential data for PBW in beneficiary cards and registers.
∎ Ensure that records on beneficiary cards and in registers are legible, complete, and consistent.
∎ Ensure registration of the PBW in appropriate services i.e. Blanket Supplementary Feeding Program (BSFP) or Targeted Supplementary Feeding Program (TSFP).
∎ Supervise and train the assigned volunteer, ensuring accurate measurement of the PBW.
∎ Provide on-the-job training (OJT) to enhance the volunteers' ability to manage data collection and registration activities effectively.
∎ Work with the Community Nutrition Supervisor (CNS) to report on absentees and defaulters, facilitating follow-up actions like home visits.
∎ Ensure provision of Iron Folic Acid (IFA) supplementation after cross checking with the health card provided from the health facility.
∎ Maintain strong coordination with health facilities to ensure appropriate Ante-Natal Care (ANC) and/or Post-Natal Care (PNC) services.
∎ Responsibility 5: Data Management and Reporting
∎ Accurately document all assessments, treatments, referrals, and follow-up actions for each infant and PBW, completing records with details on symptoms, diagnoses, treatment plans, and referrals.
∎ Update infant and PBW records with anthropometric and clinical data at each visit to monitor growth progress and support clinical and nutritional decisions.
∎ Maintain organized records of daily attendance, tracking absentees, defaulters, cured cases, deaths, and transfers.
∎ Coordinate with the Nutrition Supervisor – Nutritionist (NSN) to ensure accurate data collection and reporting, including the preparation and submission of daily, weekly, and monthly reports on infant and PBW assessments, referrals, and treatment outcomes.
∎ Store and share beneficiary data with relevant project staff as needed to support coordinated care and program efficiency.
∎ Follow up home visit (defaulter/absentee/relapse/weight static or faulter).
∎ Keep up to date and requisitions on demands all IEC materials, form format and registers books.
∎ Support digitalization database and reporting tools.

∎ Actively participate in team meetings and collaborative case reviews with staff from all facility corners.

∎ This Nurse position is responsible for the management of <6 months infants and Pregnant and Breastfeeding Women (PBW). As per the updated WHO guideline in 2023 and as part of continuum of care the management of <6 months infants will be strengthened through multiple steps of assessments. This position will be responsible for these assessments and ensuring effective referral to the relevant components. Furthermore, the nurse will be responsible for the management of PBW through CMAM approach and to ensure IFA supplementation to the PBW, when needed
