Responsibilities & Context:

Job Purpose

To provide safe, ethical, evidence-based, and patient-centered nephrology services, including diagnosis and management of kidney diseases and supervision of dialysis services. The Specialist will support the Nephrology & Dialysis Center in delivering high-quality inpatient, outpatient, emergency, and dialysis care in accordance with hospital policies and applicable Bangladesh medical standards.

Key Responsibilities

1. Clinical Care

  • Examine, diagnose, and manage patients with renal and related medical conditions.

  • Provide outpatient consultation and follow-up for patients with chronic kidney disease (CKD).

  • Manage acute kidney injury (AKI), chronic kidney disease, nephrotic syndrome, glomerular diseases, diabetic kidney disease, hypertensive nephropathy, electrolyte abnormalities, and acid-base disorders.

  • Provide nephrology consultation for admitted patients and patients referred from other departments.

  • Review investigations, including renal function tests, electrolytes, urinalysis, imaging, and other relevant diagnostic reports.

  • Develop individualized treatment plans and monitor patients' clinical progress.

  • Ensure appropriate adjustment of medications according to renal function.

2. Dialysis Services

  • Assess patients for initiation and continuation of renal replacement therapy.

  • Prescribe and supervise hemodialysis according to individual patient requirements.

  • Monitor patients before, during, and after dialysis.

  • Identify and manage dialysis-related complications, including hypotension, hypertension, cramps, arrhythmia, electrolyte abnormalities, infection, and fluid imbalance.

  • Review dialysis adequacy and relevant laboratory parameters.

  • Monitor and manage dialysis access-related problems and coordinate with appropriate surgical/interventional services when required.

  • Participate in the management of patients requiring urgent/emergency dialysis.

  • Ensure appropriate documentation of dialysis prescriptions, treatment progress, complications, and follow-up.

3. Inpatient & Emergency Responsibilities

  • Provide nephrology consultation for critically ill and hospitalized patients.

  • Participate in management of AKI, fluid and electrolyte abnormalities, acid-base disorders, and renal complications of systemic diseases.

  • Coordinate with ICU, Emergency, Cardiology, Endocrinology, Urology, Surgery, and other departments as required.

  • Attend urgent nephrology/dialysis-related cases according to hospital duty and on-call arrangements.

4. Patient & Family Counseling

  • Explain diagnosis, treatment options, prognosis, and follow-up requirements to patients and their attendants.

  • Counsel patients regarding CKD progression, medication compliance, fluid restriction, dietary requirements, blood pressure and diabetes control, and dialysis.

  • Educate eligible patients regarding different renal replacement therapy options, including hemodialysis, peritoneal dialysis, and kidney transplantation.

  • Maintain professional, respectful, and compassionate communication with patients and families.

5. Infection Prevention & Patient Safety

  • Ensure strict compliance with infection-prevention and control measures in the dialysis unit.

  • Follow hospital policies for dialysis-unit hygiene, sterilization/disinfection, biomedical waste management, and prevention of blood-borne infections.

  • Promote patient safety and promptly report adverse events and clinical incidents.

  • Ensure appropriate screening, isolation, and infection-control practices for dialysis patients in accordance with hospital protocols.

6. Documentation & Hospital Compliance

  • Maintain accurate patient records, consultation notes, dialysis prescriptions, progress notes, and discharge summaries.

  • Ensure proper documentation of informed consent and relevant procedures.

  • Maintain patient confidentiality and comply with hospital policies and applicable laws and regulations in Bangladesh.

7. Quality Improvement & Administration

  • Participate in clinical audits, quality-improvement activities, morbidity and mortality reviews, and departmental meetings.

  • Assist in developing and updating nephrology and dialysis clinical protocols and SOPs.

  • Monitor quality indicators of the dialysis service and recommend corrective actions where necessary.

  • Support appropriate utilization of dialysis resources, medicines, investigations, and equipment.

  • Participate in hospital accreditation and regulatory compliance activities when required.

8. Teaching & Professional Development

  • Provide clinical guidance and teaching to junior doctors, nurses, and dialysis staff.

  • Participate in case discussions, CME programs, clinical meetings, and departmental training.

  • Keep updated with current nephrology and dialysis guidelines and evidence-based practices.

9. Duty & Working Arrangement

  • The Specialist will perform duties six (6) days per week as per the hospital-approved duty roster.

  • Provide inpatient, emergency, and dialysis services as assigned.

  • Participate in dialysis rounds, nephrology consultations, and emergency/on-call duties.

  • Attend urgent nephrology and dialysis-related cases, including emergency dialysis, as required.

  • Working hours and on-call duties will be determined according to departmental requirements and hospital policy.

  • May be required to work extended hours, weekends, or public holidays as per the approved duty roster.

  • Ensure proper patient handover, documentation, and continuity of care.
