Responsibilities & Context:

Main Purpose: Provide comprehensive sexual and reproductive health (SRH) care to women throughout every stage of their lives, including antenatal care (ANC), intrapartum care, postnatal care (PNC), family planning (FP), and comprehensive and inclusive care for survivors of sexual and gender-based violence (SGBV). Ensure continuous follow-up before, during, and after delivery, in full compliance with MSF obstetrical and reproductive health protocols, universal hygiene practices, and newborn care standards.

Accountabilities:

• Ensuring the implementation and continuity of antenatal and postnatal care, family planning, obstetrical care (BEmONC 1 and CEmONC 2), neonatal and comprehensive abortion care, management of survivors of sexual violence, reproductive tract infections and Fistula prevention and care in accordance with MSF Reproductive Core Package of Activities and reinforce the implementation of standardized protocols.

• Where PMTCT is implemented, ensuring implementation of the PMTCT protocol in the ANC/Delivery and PNC consultations (pre counselling, test and post counselling).

• Collaborating with the medical doctor and /or nurse in the management of Sexual Violence and Intimate Partner Violence cases.

• Assessing the feasibility for referral of pregnant women from the TBAs to the SRH OPD & IPD for medical evaluation and/or complicated deliveries, when applicable.

• Ensuring hygiene and sterilization criteria (including universal precautions) are met according to MSF specifications.

• Performing cleaning and minor maintenance for biomedical equipment used. Following the user manual and protocols and alert supervisor in case of malfunctioning.

• Guaranteeing a regular and ongoing supply of drugs and equipment required for maternity activities (including monitoring/consumption control/ordering of orders).

• Properly following up all newborn babies, according to MSF guidelines, from delivery until discharge, informing mothers and relatives about importance of breast feeding, vaccination and possible complications resulting from harmful traditional practices.

• Ensuring patients centered care keeping their right of privacy and confidentiality in all consultations and procedures.

• Supervising that administrative procedures of admissions and hospitalizations comply with MSF protocols, as well as verify patients are properly informed and receive the documents required (birth certificate, vaccination card, etc).

• Participating in the organization of the ward in collaboration with other midwifes and the maternity ward supervisor. Ensure proper handover and communication with transfer of relevant information to the next shift team (especially identifying risk cases)

• According to MSF protocols: conduct normal deliveries autonomously; identify and manage obstetric emergencies; conduct referral when is needed minimizing delays; provision of newborn care with early identification of any abnormalities and infections to timely and successful intervention; be accountable for the proper request, consumption, and conditions of storage of drugs; pharmacy stock levels frequently updated; adequate plan and provision of obstetric material ensuring availability according to the rational of patients

1 BEmONC = basic emergency obstetric and neonatal care = Administration of antibiotics, oxytocics, anticonvulsants, manual removal of the placenta, removal of retained products following abortion, assisted vaginal delivery, preferably with vacuum extractor and newborn care including neonatal resuscitation.

2 CEmONC = comprehensive obstetric and neonatal care = the full package of BEmONC Plus; surgery (caesarean section, hysterectomy, laparatomy), safe blood transfusion and care to sick and low birth weight newborns

MSF Section/Context Specific Accountabilities

• Provide care in SRH outpatient, SRH inpatient and SGBV departments, working schedule varies based on department and roster needs: 12-hour shifts, nights, weekends and public holidays are expected

• Work in alignment with MSF’s principles of Respectful Maternal Care and Person-Centered Care

• Ensure the implementation and continuity of family planning services, safe abortion/ menstrual regulation care, post-abortion care, inclusive management of survivors of sexual violence, reproductive and urinary tract infections, menstrual health issues, and fistula care, in accordance with MSF’s Reproductive Core Package of Activities. Reinforce adherence to standardized MSF clinical guidelines and protocols.

• Collaborate closely in the management of sexual and gender-based violence (SGBV) cases, providing comprehensive and inclusive support, ensuring effective coordination and referral pathways with other departments and partners.

• Early identification of need for patient referrals and manage all pre-referral procedures to guarantee timely and appropriate care.

• Maintain hygiene and sterilization standards and conduct cleaning and minor maintenance of biomedical equipment to ensure safe and effective functioning.

• Always uphold patients’ rights to privacy and confidentiality, while supervising administrative and documentation procedures.

• Oversee the rational use of medications, facilities, and medical materials, ensuring appropriate stock levels and minimizing waste.

• Provide labor care and conduct NVD autonomously and instrumental vaginal deliveries with a team, according to MSF protocol.

• Manage complicated pregnancies and deliveries, such as pre-eclampsia/eclampsia, PPH, prolonged labor according to MSF standards.

• Communicate with patients and patients’ attendants and family members in calm and explanatory manner

• Manage regular and high-risk ANC & PNC,

• Provide comprehensive newborn care; monitor and evaluate newborns health status, identify risk factors and escalate their care if needed, involving Midwife Supervisor / Midwife Activity Manager / Doctor.

Contract and Benefit: 2-12 month- contract renewable based on performance and operational needs. Salary according to Level Six (6) of the internal MSF salary grid (BDT 85,5000 Gross). Salary is not negotiable. Income tax deduction from salary based on current income tax rule. 2 annual leave days per month worked. Medical insurance for employee and direct dependents based on Health Care Policy- Locally Recruited Staff.

Others: There is no provisions for relocation. 2 Festival Bonuses after completion of one year of uninterrupted continuous service.
