Educational Requirements:
∎ Bachelor Degree in Social Science or related disciplines with excellent academic result. Experiences in TB field in national or international non-profit organizations will get preference.

∎ Position Profile:
∎ Field Organizer is responsible for the day to day field activities along with carrying out the proper program implementation plan at the designated diabetic service delivery points and area. This description in not exclusive and can be adapted as per the need of the program, without the obligation of a complete revision.
Job Responsibilities:
∎ He/she will be responsible for maintaining proper liaison with data collectors for proper screening of the diabetic patients regarding TB at the center.
∎ He/she will be responsible for maintaining proper liaison with data collectors for referral of TB presumptive cases to the nearby TB service delivery points.
∎ He/she will be responsible for ensuring registration of the diagnosed TB patients.
∎ He/she will be maintaining a periodic communication with the patients through field visit.
∎ He/she will be responsible for maintaining proper liaison with data collectors for updating and maintaining the documentation registers at the center level.
∎ He/she will be responsible for ensuring follow up visits of the diagnosed TB-DM patients to the doctors at affiliated and sub affiliated centers for proper management.
∎ Ensure proper display of ACSM materials at Affiliated, sub affiliated Association and service delivery points under him.
∎ Keep the appropriate records on all activities (patient register, presumptive registers, activity records etc.) and reports as per instructions. Prepare a work plan synchronize with the program implementation plan.
∎ Collect monthly reports from Data Collectors with proper validation and send the cumulative report to Regional/Area coordinators within the time frame.
∎ Participate in project-meeting and seminar as required.
∎ He/she will responsible to conduct a designated number of field visit to identify new cases, conduct contact tracing and identify the treatment outcome of the diagnosed patients within the limits of available time and accessibility.
∎ Maintain proper liaison with the Govt. and PNGOs clinics and other stakeholders through monthly visits.
∎ Identify the gaps in the service delivery if any and share it with upper authority through Area coordinators for proper management and strengthening. Do every effort possible, within the limits of the applicable instructions, to assure a good cooperation with all the health personals. In case of an impasse inform the respective area coordinator and/or program management unit for a fruitful resolution.
∎ Any other duties assigned by the authority.
Salary:
∎ As per negotiation.
