Responsibilities & Context:
To maintain files confidentially for all inpatient, ex-patient & other patients concerned.
To send newsletters and follow up forms to discharged patients and keep in touch with discharged patients for life.
Find out about patients’ social, financial, mental and physical circumstances help and motivate them to resolve their problem.
To arrange for external investigations for patients.
To arrange patients’ recreational activities, education and cultural functions.
To motivate relatives to take responsibility for patients.
To attend patients’ meetings, monthly staff meeting, ward round, departmental meeting and conferences on disability.
To organize and attend departmental meeting.
To maintain departmental equipment and record keeping.
To contacts relatives of seriously ill and deceased patients and make funeral arrangements for deceased without relatives.
Arrange internal investigation for patients.
Arrange blood for poor and emergency patients.
To organize awareness programme on disability such as road safety campaigning (school, college, madrasa etc.).
Responsible to lead the outreach program with all department representatives.
Responsible for patient’s home visit.
To arrange patient financial assessment at the time of admission or any support.
Any other tasks as assigned by the supervisor.
