Job Context:
∎ Adult Day Activities Center- Shopno, is a holistic space for the elder lies to enhance the quality of life and well-being of older people and thus promote active ageing and care of the aged through the life-course. We are committed to providing family caregivers with respite and attempt to directly improve the lives of the elderly themselves. The program is anchored in components of wellness model which aims to build functional independence, quality of life and social participation, rather than dependence on services.
∎ The key components of the wellness model include: An emphasis on capacity building or restorative care to maintain or promote a client`s capacity to live as independently as possible. The overall aim is to improve functional independence, quality of life and social participation. An emphasis on a holistic person-centered approach to care, which promotes clients` wellness and active participation in decisions about care.Provision of more timely, flexible and targeted services that are capable of maximizing the client`s independence.
∎ The center offers range of high-quality activities and services ensuring: Health, nutritional, social, and daily living needs support in professionally staffed, group settings. Wide array of activities services to address the range of conditions and needs presented by participants and their caregivers. Access to necessary support and opportunity to establish relationships and connections. Provide safety and structure for participants either in physical environment or the program design of adult day activities.

Job Responsibilities:
∎ Lead and encourage participants to join in meaningful activities whilst maintaining their independence, choice and dignity, to enable them to lead as full and active life as possible.
∎ Create a stimulating and varied program of leisure activities and social events for participants to enjoy.
∎ Work with each new participant to understand their likes/dislikes, abilities and needs to put together an individual care plan and to evaluate regularly.
∎ Engage the local community into the daily lives of participants by initiating contact with local community groups enabling participants to partake / continue with hobbies and interests as desired.
∎ Recruit a network of volunteers to provide a variety of desired appropriate social opportunities as identified by the participants.
∎ Consult with each participant in order to identify their social and recreational requirements according to their likes/dislikes, abilities and personal preferences.
∎ Ensure the well being and care plan for each participant is in place within 2 weeks from the date of admission and it is kept up to date and regularly reviewed.
∎ Ensure that all participants receive some form of social activity each week and participants receive one to one time, particularly those that do not wish to participate in group activities. This should include opportunities within the center and the wider community.
∎ Liaise and consult with staff and relatives regarding the well being and care of participants and particular activities that may be beneficial and stimulating.
∎ Formulate and maintain a stimulating program of activities for groups of participants and for individuals whilst considering participants particular likes and dislikes and abilities as appropriate.
∎ Produce a weekly program of activities and social events and ensure it is advertised within the center and communicated to all participants.
∎ Liaise with relatives and staff regarding special individual requirements, such as birthdays and anniversaries, cultural and religious festivals and celebrations, and to be aware of the religious calendar that may affect activity pursuits, relevant to the ethnic mix of participants.
∎ Familiarize and connect with all key community groups whose particular expertise would make an invaluable contribution to the day center. To support any volunteers in their role and ensure they receive appropriate induction, training, supervision and support.
∎ Promote and enhance the existence of the center in the community and encourage the use of communal spaces within the center for groups to meet and hold events.
∎ Regularly assess the effectiveness of the social events and activities program
∎ Work as an active team member with your colleagues within the center to ensure the highest standards of care are achieved.
∎ Undertake any reasonable request which facilitates the participant's right to choose, dignity and respect.
∎ Respect the confidentiality of any matters they may learn in the course of their duties relating to members, staff and the general public as well as matters of business concerning the Project.
∎ Provide progress report and status to line manager
∎ Continuous personal development related to new job context and challenges
∎ Update continuously on technology
∎ Maintain quality standards and ensure health and safety compliance

Additional Requirements:
∎ Age 30 to 40 years
∎ Both males and females are allowed to apply
∎ Having experience in a relevant field, for example a social work, development, field operation
