∎ 2 to 3 years’ experience in hospital pharmacy or any pharmacy shop
∎ Education / License Required: HSC (Science) (training in health science or drug handling) /Diploma in Pharmacy / Diploma in health science). A registered Diploma Pharmacist grade-B from Bangladesh Pharmacy Council
∎ To ensure smooth function of the Dept. Liaises with: Hospital/Medical Staff
∎ Report to: Head of Pharmacy, Pharmacy Advisor, Pharmacy Assistant Incharge, and Pharmacist.
∎ Financial Responsibility: Nil
∎ Ability to work as a team member

Responsibilities & Context:
∎ Internal: Medical, Nurses Services Dept., pharmacy staff, Laboratory, Radiology, Bio-medical Engineering, Housekeeping, Support service department, CSSD, security and others
∎ Responsible for a number of technical and clerical skills on the delivery of pharmaceutical support under the direction and supervision of a pharmacist/incharge in accordance with the Farazy Hospital departmental policies and procedures and to provide a safe and efficient standard of patient services. The job involves working shifts.
∎ Responsible for transcription of the prescription order and checking the accuracy of Quantity, Strength, Dosage.
∎ Correctly enter the medicines to patient head through computer system with the Phone Number (which have been dispensed to him/her for proper billing.
∎ Enter data & labeling information regarding prescription order form.
∎ Print out packing and labeling information. Stick drug label onto plastic bag and packing the
∎ Medicine as per request / doctors' order
∎ Check the label against the drug order for correct medication, strength, quantity etc. before
∎ dispensing to Patient.
∎ Responsible for reporting daily inventory to pharmacy in charge.
∎ Responsible for daily sales report.
∎ Perform any task as requested by pharmacists/ in charge.
∎ Responsible for dispense the medicine when needed.
∎ Maintain adequate stock of medications and supplies in the working area (OPD) through requisition (by computer and paper) from pharmacy before finishing your duty.
∎ Deliver medications to the nursing units/stations.
∎ Answer the telephone and inquiries as necessary.
∎ Arrange pharmacy stock onto shelves cabinets and refrigerator maintaining proper temperature.
∎ Check the expiry date of the medicines in OPD on your responsibilities.
∎ Check the temperature of the refrigerator morning and evening daily and record it on the sheet.
∎ Ensure that all stock in the pharmacy is "in date" package, all items going to expire (3-6 months before) or expired items are returned to the warehouse for proper disposal as per hospital policy.
∎ Perform other applicable tasks and duties assigned within the realm of his/her knowledge, skills and abilities
