Experience

Knowledge

Essential – As per MD1400 and for the focal point as follows:

• Have a working understanding of key pathogens, their usual antibiotic susceptibilities, typical clinical presentations, and empiric or targeted treatment.

• Have a working understanding of the modes of action of antimicrobials including their spectrum of activity.

• Have a working understanding of the key principles of infection prevention and control (IPC) and the relationship between IPC and AMR Antimicrobial Resistance).

• Have a working understanding of the appropriate use of antimicrobials to prevent the emergence of resistance and avoidance of adverse effects.

• Have a working understanding of the use of diagnostic tests in the investigation and management of infectious diseases. How to use data for feedback and reporting.

• Computer literacy – Word and Excel and Power Point.

Desirable:

• Understanding of behaviour change principles.

• Able to provide face to face feedback to prescribers about antibiotic prescriptions.



Responsibilities & Context:

MD tasks as per MDO1400. In addition, for Antimicrobial Stewardship Focal Point, the following apply:

• Leading, promoting and supporting antimicrobial stewardship efforts in the healthcare facility(s) across the spectrum of healthcare.

• Establish and participate in a multidisciplinary AS committee (that meets regularly with Terms of Reference).

• Identify antimicrobial stewardship priorities together with the AS committee and the mission.

• Monitor, collect and analyze the antimicrobial data reported to the multidisciplinary AS committee.

• Conduct regular audits relevant to stewardship priorities. This might include audits of antibiotic use, treatment of select conditions, indications for microbiology testing, and communication of results.

• Provide feedback to the project and the mission regarding results of audits and plan improvements.

• Co-ordinate and deliver education to other medical, nursing and pharmacy staff around stewardship.

• Support other clinical staff regarding antibiotic choices and regularly participate in clinical rounds.

• Liaise with the laboratory for timely communication of results (where relevant), interpretation of results and feedback to

clinicians.

• Collaborate with lab + IPC to prospectively monitor pathogens of concern and respond to hospital outbreaks including outbreaks of multidrug resistant organisms/bacteria (MDRO).

• Identify opportunities to improve stewardship in the project (e.g., therapeutic drug monitoring, OPAT).

• Participate in relevant project level committees (e.g., IPC, Quality improvement, Morbidity + Mortality reviews).

• In collaboration with IPC and the clinical team, participate in detection, management and prevention of healthcare associated infections. MSFKutupalong (KTP) secondary hospital (KTP) has approximately 113 inpatient beds capacity, a 24/7 ER and 6 days/week OPD (which includes mental health and psychiatric care). Inpatient departments include adult IPD, Isolation, Maternity, Pediatrics, and Neonatal. KTP hospital also supports referral to tertiary hospitals. with: Maternity, Pediatrics, Neonatal, adult IPD, SGBV, Isolation, DTC, ER 24/7, OPD 6 days/week (including Mental Health & psychiatric care) and referral services. The AMS MD responsibility encompasses all services where antimicrobials are prescribed.

Diagnostic services include an X-ray department, point-of-care ultrasound (POCUS) and laboratory, which provides standard hematology and urine tests, some serological tests (mostly RDT-based) and culture and sensitivity tests (blood and sterile fluids) through Stepwise Optimization of Laboratory Antimicrobial Susceptibility testing (SOLAS) and further processing in Dhaka.

Lead and support antimicrobial stewardship efforts in patient care throughout all hospital services

- Collaborate with all clinicians and prescribers to ensure the use of appropriate diagnostic tools and prescription of antimicrobials in line with MSF guidelines or culture results

- Ensure a tailored approach to antimicrobial treatment when culture results are available, support de-escalation of antimicrobials when indicated

- Contribute to daily rounds in different IPDs and participate in major ward rounds, providing input and feedback from an AMS perspective

- Closely follow up on culture and sensitivity results – both SSOLAS and definitive results, and ensure that results are acted upon as early as possible

- Reviews and validates the use of restricted antimicrobials, checks that prescription is justified and followed up in a timely manner

- Promote adherence to infection prevention and control measures in all patient-care areas

- Collaborate with MAMs/HCL for referral of patients outside the project

- Collaborate with other team members (IMS specialists, MAMs, nursing managers and supervisors) for optimization of surgical procedures and wound care from an AMS perspective, focusing on prevention of colonization and secondary infection and appropriate management when infections are identified.

- Collaborate with a multidisciplinary team throughout the hospital, including but not limited to: infection prevention and control (IPC), pharmacy, laboratory, managers and supervisors.

Support the laboratory team

- Ensure timely request of cultures based on the clinical indications

- Support correct blood culture sampling by different teams

- Review and analyze culture contamination rates, support improvement plans where relevant

Support collection and analysis of AMS-related data

- Conduct monthly reviews of blood culture results (interpretation of true pathogens vs. contaminants, turnaround time, resistance patterns, etc.) and contribute to the monthly medical report (MMR).

- Monitor culture results for infection and resistance patterns and potential outbreaks

- Coordinate a global point prevalence survey (GPPS) and participate in other audits (such as prescription audits);

support analysis, present results and give recommendations

- Support MAM and HCL in consumption analysis of antimicrobials and provide recommendations for the international medical order (IMO)

Collaborate with other team members in case of hospital-acquired outbreaks, especially outbreaks of multidrug resistant organisms (MDROs)

- In collaboration with hospital management team (HMT), infection prevention and control (IPC), managers and supervisors, advisors and relevant healthcare staff ensure that an appropriate outbreak response is implemented in line with MSF guidelines

- Monitor and report on ongoing outbreaks; collaborate with IPC team to prepare a report once the outbreak is concluded

Conduct and deliver trainings, focused on antimicrobial stewardship, but may include other relevant infectious disease topics as needed (e.g. tuberculosis)

Support the development of and implement project-specific antimicrobial guidance or documents

Participate in research implemented in the project

Co-chair the Antimicrobial Stewardship Committee

- Review terms of reference if needed, ensure routine monthly meetings are held and outcomes shared with committee members and other team members, as appropriate

- Participate in the IPC committee

Routine meetings with AMS Advisor(s) and others as relevant – provide updates, discuss project AMS efforts, provide feedback, consult on difficult cases, etc.

Use and support the use of Telemedicine for second opinions or sub-specialistic support in the management of complicated patients, or consult with Advisors, as needed

Conduct other/external collaboration related to AMS

- Collaborate with the HPCE (Kutupalong and Balukhali) and Advocacy teams for community engagement and advocacy efforts on AMS

- Collaborate with relevant counterparts at other sections (especially OCP) in relation to AMS, outbreak responses, etc.

Coordinate activities for the World Antimicrobial Awareness Week (18 to 24 November)

Practice and promote person-centered care principles and support improvements in quality of care (QoC).

    This is a Permanent Contract with Indefinite contract duration. Benefit

    • Salary according to Level 10 of the internal MSF salary grid (BDT 161000/- gross, before seniority and income tax deduction).

    • 2 annual leave days per month worked.
