Experience:

Working Conditions
The position will be based at a Primary Health Care Centre (PHCC) in the Rohingya Response, Cox’s Bazar.
The role may involve working on a roster or shift basis, including weekends, public holidays, or emergency duty, depending on operational requirements.
The position holder will work closely with clinical, programme, supply chain, and other support teams.
The role involves working in a challenging and fast-paced humanitarian healthcare environment.
The position holder may be required to support pharmacy services during disease outbreaks, natural disasters, fires, mass casualty incidents, or other humanitarian emergencies.

Position Summary
The Pharmacist will be responsible for ensuring the safe, effective, timely, and rational management and dispensing of medicines and medical supplies at the Primary Health Care Centre (PHCC) in the Rohingya Response. The position holder will ensure proper dispensing, patient counselling, medicine storage, stock management, inventory control, documentation, maintain digital consumption tool, and quality compliance with approved national & WHO guidelines, organizational policies, GMT guidelines and relevant health sector standards.
The Pharmacist will work closely with Medical Officers, Nurses, Midwives, and other relevant healthcare staff to ensure the continuous availability and appropriate use of essential medicines and medical supplies, while maintaining high standards of patient safety, quality of care, and accountability.
KEY AREAS OF ACCOUNTABILITY:
Pharmacy Service & Medicine Dispensing
Ensure safe, accurate, timely, and appropriate dispensing of prescribed medicines to patients.
Review prescriptions for completeness, appropriateness, dosage, frequency, duration, and potential medication-related concerns within the scope of practice.
Verify patient details and ensure that the correct medicine, strength, dosage, and quantity are dispensed.
Provide clear counselling to patients and caregivers regarding medicine use, dosage, administration, duration, possible side effects, precautions, and storage.
Ensure proper labelling and packaging of medicines before dispensing.
Maintain confidentiality and ensure respectful, equitable, and patient-centered pharmacy services.
Promote patient understanding and adherence to prescribed treatment.
Prescription Review & Rational Use of Medicines
Support the rational and appropriate use of medicines in accordance with approved treatment guidelines and protocols.
Identify potential prescription errors, inappropriate doses, duplication, contraindications, or other medicine-related concerns and communicate with the prescriber for clarification.
Promote appropriate use of antibiotics and other priority medicines.
Support measures to reduce medication errors and improve patient safety.
Participate in activities related to rational medicine use, Pharmacovigilance and ADR reporting, antimicrobial stewardship, and quality improvement as applicable.
Medicine & Medical Supply Management
Maintain appropriate stock levels of medicines and essential medical supplies required for PHCC services.
Monitor stock availability and ensure timely identification and reporting of shortages or potential stock-outs.
Receive, inspect, verify, and properly document medicines and medical supplies received by the pharmacy.
Ensure appropriate storage conditions according to manufacturer requirements and organizational procedures.
Maintain proper arrangement of medicines using applicable stock management principles, including First Expiry, First Out (FEFO).
Regularly monitor expiry dates and take appropriate action to prevent the dispensing of expired or damaged medicines.
Ensure safe segregation and management of expired, damaged, recalled, or unusable medicines according to approved procedures.
Conduct regular physical stock counts and reconcile stock balances with stock records.
Inventory Control & Documentation
Maintain accurate and up-to-date pharmacy stock cards, bin cards, registers, and other required inventory records.
Ensure timely recording of medicine receipts, issues, transfers, adjustments, losses, expiries, and returns.
Conduct routine stock reconciliation and investigate discrepancies.
Maintain proper documentation of controlled or restricted medicines, where applicable, according to relevant policies and regulations.
Support periodic inventory checks, audits, and physical verification of pharmacy stock.
Ensure all pharmacy records are complete, accurate, and available for review.
Patient Counselling & Medication Adherence
Provide appropriate counselling to patients and caregivers to ensure the safe and effective use of prescribed medicines.
Explain dosage schedules, methods of administration, duration of treatment, possible side effects, and precautions.
Support patients with chronic diseases to understand the importance of treatment adherence and regular follow-up.
Provide appropriate counselling on the safe storage of medicines, particularly for children and vulnerable patients.
Refer patients to the relevant clinical team when medicine-related concerns or adverse effects require medical assessment.
Clinical Coordination & Multidisciplinary Teamwork
Work closely with Med-log team, Medical Officers, Senior Medical Officer Lead, Clinical In-Charge, Nurses, Midwives, Laboratory, Nutrition, MHPSS, and other relevant teams.
Communicate regularly with prescribers regarding medicine availability, alternative treatment options, and medicine-related concerns.
Support continuity of care by ensuring the availability of prescribed medicines within the approved PHCC service package.
Participate in clinical meetings, case discussions, pharmacy meetings, and programme review meetings as required.
Communicate pharmacy-related challenges, stock concerns, and patient safety issues to the appropriate supervisor.
Quality Assurance & Patient Safety
Ensure pharmacy services are delivered in accordance with approved national guidelines, standard operating procedures, and organizational policies.
Maintain high standards of accuracy and quality during prescription review and medicine dispensing.
Identify and report medication errors, near misses, adverse drug reactions, or other patient safety concerns according to applicable procedures.
Participate in pharmacy audits, supportive supervision, and quality improvement activities.
Support the implementation of corrective actions to address identified gaps or risks.
Promote a culture of patient safety, accountability, and continuous quality improvement.
Infection Prevention & Control (IPC)
Follow and promote Infection Prevention and Control procedures within the pharmacy.
Ensure appropriate hand hygiene and standard precautions during pharmacy operations.
Maintain a clean, safe, and organized pharmacy environment.
Follow relevant procedures for handling medicines and supplies during disease outbreaks or public health emergencies.
Identify and report potential IPC risks or gaps within the pharmacy.
Support the implementation of IPC measures during outbreaks and other humanitarian emergencies.
Health Information, Reporting & Data Management
Maintain accurate records of medicine consumption, stock status, dispensing activities, expiries, and losses.
Prepare and submit daily, weekly, monthly, and other required pharmacy reports.
Support forecasting and quantification of medicine and medical supply requirements based on consumption and service utilization.
Provide timely stock status information to the supervisor and relevant programme teams.
Work closely with the relevant Supply Chain, Logistics, MIS, and programme teams to ensure the accuracy and timeliness of pharmacy data.
Maintain confidentiality of patient, medicine, and programme information.
Coordination with Supply Chain & Procurement
Coordinate with the Supply Chain and relevant programme teams to ensure the timely replenishment of medicines and medical supplies.
Support the preparation and submission of medicine requisitions based on approved consumption data and stock requirements.
Monitor pending orders and follow up on delayed or incomplete supplies.
Provide timely information regarding stock shortages, near-expiry items, slow-moving items, and urgent requirements.
Support proper receipt, storage, distribution, and accountability of medicines and medical supplies.
Emergency Preparedness & Response
Maintain adequate preparedness to support pharmacy services during disease outbreaks, fires, natural disasters, mass casualty incidents, and other humanitarian emergencies.
Support the management and availability of essential emergency medicines and medical supplies.
Participate in emergency preparedness planning, drills, and response activities as required.
Ensure continuity of essential pharmacy services during emergencies.
Remain flexible to undertake additional pharmacy-related responsibilities based on emergency and programme requirements.
