Responsibilities & Context:
1. Identification and Registration of Beneficiaries/Survivors:
Identify potential beneficiaries/survivors through community outreach, field visits, partner organizations, and referral mechanisms.
Conduct initial screening to determine eligibility according to project criteria.
Register beneficiaries using approved intake forms and digital or manual databases.
Ensure informed consent is obtained from survivors before collecting personal information.
Maintain accurate and updated beneficiary records.
2. Conduct Needs Assessment and Prepare Case Management Plans:
Carry out comprehensive assessments to understand survivors’ social, psychological, legal, medical, and economic needs.
Develop individualized case management plans based on the assessment results.
Prioritize urgent protection concerns and immediate support requirements.
Ensure that case plans are developed in consultation with survivors and reflect their choices and best interests.
3. Coordinate Support Services and Referrals:
Facilitate access to appropriate services, including medical care, psychosocial support, legal aid, shelter, livelihood support, and protection services.
Establish and maintain referral pathways with relevant service providers such as NGOs, government agencies, healthcare facilities, and legal institutions.
Coordinate with service providers to ensure timely and effective support for survivors.
Track referral outcomes and ensure beneficiaries receive the services they require.
4. Monitor Beneficiary Progress and Conduct Follow-ups:
Conduct regular follow-up visits or communications with beneficiaries to monitor progress against case plans.
Update case management plans based on evolving needs and circumstances.
Ensure beneficiaries are receiving adequate and appropriate services.
Document improvements, challenges, and outcomes of each case.
5. Maintain Case Documentation and Reporting:
Maintain organized and confidential case files, including intake forms, assessment reports, case plans, referral forms, and follow-up notes.
Update case management databases regularly.
Prepare periodic case management reports for project management and donors.
Ensure data accuracy, quality, and timely submission of reports.
6. Ensure Confidentiality, Safeguarding, and Survivor-Centered Support:
Strictly maintain confidentiality and privacy of survivor information.
Follow safeguarding policies and Protection from Sexual Exploitation and Abuse (PSEA) guidelines.
Apply a survivor-centered approach that respects dignity, safety, and non-discrimination.
Ensure informed consent and respect beneficiaries’ decisions throughout the case management process.
7. Coordination with Stakeholders and Service Providers:
Maintain close coordination with government departments, NGOs, legal service providers, healthcare institutions, and community leaders.
Participate in coordination meetings, case conferences, and protection networks.
Strengthen partnerships to improve referral mechanisms and service delivery.
Advocate for survivors’ access to essential services and protection mechanisms.
8. Capacity Building and Awareness (Optional but often included:
Support community awareness sessions on protection issues and available services.
Assist in training community volunteers and stakeholders on referral pathways and survivor support.
9. Other Responsibilities:
Support project monitoring and evaluation activities.
Contribute to project documentation, success stories, and learning reports.
Perform any other duties assigned by the Project Manager or Program Coordinator.
