Requirements:

Responsibilities & Context:
∎ Medication Management:
∎ Dispense prescribed medications accurately to inpatients and outpatients.Review prescriptions for appropriateness, dosage, and potential drug interactions.Prepare sterile and non-sterile compounds when necessary.
∎ Clinical Support:
∎ Collaborate with physicians, nurses, and other healthcare professionals to provide optimal pharmaceutical care.Monitor patient medication therapies and provide guidance to improve outcomes.Participate in ward rounds and contribute to patient treatment plans.
∎ Patient Education:
∎ Provide counseling to patients on medication usage, side effects, and storage.Offer guidance on adherence to therapy and answer any drug-related queries.
∎ Inventory and Compliance:
∎ Maintain inventory of pharmaceutical supplies, ensuring proper storage and handling.Ensure compliance with hospital policies, legal, and ethical standards.Participate in drug audits and pharmacovigilance activities.
∎ Documentation and Reporting:
∎ Maintain accurate records of dispensed medications and patient interactions.Report adverse drug reactions and medication errors as per protocol.
