Candidates must have completed a recognized Computer Training Course and possess a valid Computer Training Certificate.
Must have a minimum English typing speed of 40 words per minute (WPM) and Bangla typing speed of 30 words per minute (WPM).
Sound knowledge of MS Word, MS Excel, email, internet, and Hospital Information Systems (HIS) will be preferred.
Previous experience in hospital, healthcare, IPD, medical documentation, or transcription-related work will be an added advantage.
Good command of English spelling, grammar, typing, and medical terminology is preferred.
Must possess good typing accuracy, attention to detail, confidentiality, and communication skills.
Assist Doctors and Nurses in preparing and completing patient discharge documentation accurately and within the prescribed timeframe.
Prepare, type, and format Discharge Summaries, Treatment Summaries, Medical Certificates, Referral/Transfer Notes, and other IPD-related documents as instructed by the concerned Doctors.
Accurately transcribe doctors’ instructions, clinical findings, diagnoses, procedures, treatment details, medications, follow-up advice, and other relevant patient information.
Ensure the accuracy of patient demographics, medical terminology, spelling, grammar, dates, investigations, medications, and other clinical information in all documents.
Coordinate with Consultants, Medical Officers, Nursing Services, IPD staff, Front Desk, Billing, Pharmacy, and other concerned departments to facilitate a smooth and timely discharge process.
Follow up with concerned Doctors and Nurses for pending, incomplete, or corrected discharge documents to ensure timely completion.
Review completed documents for formatting, completeness, and accuracy before forwarding them for consultant review, correction, and signature, where applicable.
Maintain proper records and filing of discharge-related documents in accordance with departmental procedures and organizational requirements.
Operate Hospital PMS systems for data entry, document preparation, updating, and record maintenance.
Maintain strict confidentiality and privacy of patient information and ensure that all documents are handled securely.
Support the IPD team in maintaining the required discharge turnaround time (TAT) and identifying documentation delays or discrepancies.
Follow departmental SOPs, quality standards, documentation requirements, and organizational policies at all times.
Assist in audit, quality improvement, documentation review, and other administrative activities related to IPD services when required.
Perform any other duties related to IPD documentation and discharge coordination as assigned by the Head of Administration Services or Management.
