Responsibilities & Context:

Medication Dispensing and Patient Safety

  • Accurate Dispensing: Review and accurately dispense medications for inpatients (ward stock, individual patient prescriptions) and outpatients, ensuring the correct drug, dosage, route, and time.

  • Prescription Verification: Clinically screen and verify physician's orders/prescriptions for appropriateness, completeness, potential drug-drug interactions, contraindications, and appropriate dosing (especially for high-risk medications and those requiring dose adjustments, such as in renal or hepatic impairment).

  • Emergency Supply: Ensure the timely and correct supply of critical and emergency medications, especially during night shifts or in emergency/ICU scenarios.

  • Controlled Substance Management: Strictly adhere to protocols for dispensing, record-keeping, and storage of narcotics, psychotropics, and other controlled drugs.

Inventory and Stock Management

  • Stock Control: Monitor and maintain optimum stock levels of medications on the assigned shift, coordinating with procurement or main store as required, especially for immediate needs.

  • Storage Compliance: Ensure all drugs, including high-alert medications and those requiring cold storage, are stored correctly according to legal and departmental policies.

  • Expiry Monitoring: Proactively check and manage near-expiry and expired medications to prevent dispensing errors and financial loss.

  • Emergency Kit Management: Regularly check and replenish crash cart medications and ward emergency boxes to ensure they are always ready and non-expired.

Shift Handovers and Communication

  • Handover Report: Conduct thorough and detailed handovers at the beginning and end of each shift, highlighting pending work, critical patient alerts, unresolved medication issues, stock discrepancies, and any recent policy changes.

  • Inter-Departmental Liaison: Provide timely and accurate drug information to nursing and medical staff regarding drug availability, dosage, side effects, and administration during the shift.

  • Documentation: Maintain accurate and complete records of all dispensing, inventory movements, and pharmaceutical interventions made during the shift.

Quality and Compliance

  • Regulatory Adherence: Ensure all pharmacy operations during the shift comply with hospital policies, as well as local and national pharmaceutical laws and regulations.

  • Error Reporting: Document and report any medication errors, near-misses, or adverse drug reactions (ADRs) that occur during the shift according to hospital protocols.

  • Supervision: Supervise and guide pharmacy assistants or technicians working on the same shift, checking their work to maintain dispensing accuracy.
