Responsibilities & Context:

Community Development for Asian Americans, CDAA, is seeking to hire Care Coordinators.

The Care Coordinator is responsible for coordinating and overseeing the provision of integrated medical and behavioral health service delivery for a defined caseload of enrolled/assigned members. The Care Coordinator will be responsible for the overall management of the member’s care plan. They will provide the best quality service while coordinating patient long-term care and acting as a liaison between the agency, patient, and all vendors. In addition, the case manager must sustain constant communication between all involved parties, including but not limited to: Vendors, NCMs, HHAs, Family vendors, and any other members of the service team while adhering to CDAA’s policies and procedures.

This position provides Case Management Services to consumers to help consumers gain access to resources and services needed to meet their mental health needs, achieve recovery, and live independently in the community. Services are coordinated with internal and external providers to help each consumer achieve personalized case management needs.

Responsibilities:

  • Gather information for and/or conduct outreach, assessment, re-assessments, and care plans.

  • Provide care management to a caseload through the coordination of medical, mental health, and substance use services.

  • Participate in case conferences with health home team including, Case Managers and treatment providers.

  • Ensure entitlements, insurance, and benefits are in place.

  • Develop service plans and resolve barriers to effective service utilization.

  • Monitor the client's progress in utilizing services (appointments, treatment, medication, etc.) through telephone and direct contact.

  • Assist in crisis intervention and provide or refer to crisis services.

  • Ensure that clients follow up with aftercare discharge (i.e., fill prescriptions, make appointments).

  • Communicate with patients and their families regarding appointment scheduling, physician plan of care, appointment changes, and schedule changes.

  • Maintain patient confidentiality and privacy with dignity and respect.

Qualifications:

  • Bachelor’s degree in any discipline from a reputed university. Freshers are encouraged to apply.

  • Experience providing medical, mental health, or substance use-focused care coordination services to individuals with Serious Mental Illness, developmental disabilities, and substance use disorders, or linking those individuals to services required. Experience in criminal justice preferred.

  • Knowledge of healthcare environments, clinical terminology, and health information systems strongly preferred.

  • Excellent organizational and time management skills, people skills, verbal, and written communication skills required.

  • Must be proficient in computer applications or other automated systems such as spreadsheets, Microsoft Office applications, calendars, and email.

  • Health Home experience is a plus.

Work Hours: Monday – Friday, 7/8 PM until 3/4 AM
