Must have A-grade registration from Bangladesh Pharmacy Council (BPC).

Responsibilities & Context:
· Perform prescription audit & appropriateness review of individual patients’ medication order & report all medication error through using software.
· Provide information to doctors about any observation related to appropriateness review and prescription audit.
· Assist healthcare professionals by providing information related to medicines and meet queries of patients as and when required.
· Provide counselling on medication use, possible side effects and other necessary information to the patients.
· Indenting of ordered medications and reconciliation of medicine to avoid medication error.
· Monitor rational use of Restricted Antibiotics through Anti-Microbial Stewardship Program.
· Monitor the adverse drug reaction & medication errors and submit report as per policy.
· Monitor medication effect for individual patient and fill the drug intervention form if required.
· Monitor new drug evaluation and respond to any recall process.
· Review discharge medication orders and counsel all patients receiving discharge medication.
· Return all unused drugs through system and physically to IPD Pharmacy if doctor change or stop any drug.
· Perform monthly audit in allocated Medication Storage Area and Crash Carts to monitor stock availability facilitate near expiry management of narcotic and other medications.
· Maintain effective communication with staffs of all levels for ensuring medication services.
· Comply with hospital policy, code of conducts and all security measures and quality standards.
· Perform all other tasks related to Ward Pharmacist role as & when required by the Department.
