Skills Required:

Additional Requirements:

●         Minimum 2–3 years' clinical experience in STI management, HIV testing and counselling or primary healthcare.

●         Demonstrated experience working with key populations — gender diverse people, hijra communities and/or sex workers — or clear demonstrated commitment to this population's needs.

●         Working knowledge of national HIV/STI testing algorithms, treatment protocols and referral pathways.

●         Experience in the Rohingya response or another humanitarian/camp-based setting.

●         Training in CMR, GBV referral pathways or MHPSS basics.

●         Familiarity with SOGIESC concepts and gender-affirming care principles.

●         Working knowledge of Chittagonian dialect and/or Rohingya language.

Responsibilities & Context:

1. Clinical Service Delivery for Gender Diverse Populations

●         Provide quality STI, HTC and general health services to GDP beneficiaries and sex workers at service centres, sub-service centres and camp-level outlets.

●         Conduct physical examinations in strict adherence to Infection Prevention and Control (IPC) procedures, ensuring privacy and informed consent.

●         Take sexual and clinical histories in a non-judgmental, gender-affirming manner using the beneficiary's chosen name, adapting clinical enquiry to anatomy and sexual practices rather than assumed gender.

●         Recognise and manage or refer presentations more prevalent among GDP beneficiaries, including anorectal and oropharyngeal STIs and injection- or hormone-related complications.

●         Where informally obtained gender-affirming hormone use is reported, record sensitively, advise on risks and refer to specialist services. Do not prescribe or dispense hormones outside approved protocol.

●         Document medical complaints on health cards, prescribe per treatment protocols and BMDC guidance, and ensure correct dispensing including Directly Observed Therapy (DOTs).

●         Issue STI prescriptions for beneficiaries and partners per BMDC protocol, handling partner notification confidentially and without coercion.

2. HIV Testing and Counselling (HTC)

●         Conduct HTC as scheduled, including pre- and post-test counselling, sample collection and rapid HIV testing per the national algorithm.

●         Deliver results privately, maintaining absolute confidentiality of HIV status and gender identity.

●         Provide post-result support and, for reactive results, arrange escorted linkage to the Care, Support and Treatment Centre (CSTC) for confirmatory testing and ART enrolment; follow up to support treatment initiation and adherence.

●         Provide risk-reduction counselling using the 4Cs approach (compliance, counselling, condom demonstration, contact tracing) per national and project guidelines.

●         Counsel on and refer for PrEP and PEP, ensuring PEP referrals occur within the 72-hour window.

●         Prepare HTC reagent and consumable requisitions, maintain stock registers and the cold chain, and store Quality Control samples correctly for BSMMU submission.

3. TB Screening and Referral

●         Screen all clinic attendees for TB, with particular attention to HIV-positive and immunocompromised beneficiaries.

●         Refer presumptive TB cases promptly to DOTS centres and follow up TB-positive cases to support adherence.

4. Referral and Linkage

●         Maintain an updated referral directory covering HIV care, specialised STI care, MHPSS, GBV case management and secondary/tertiary facilities.

●         Refer to internal services including psychosocial counselling and GBV case management where clinically indicated, always with informed consent.

●         Recognise clinical indicators of gender-based violence and sexual assault and initiate the GBV referral pathway using survivor-centred principles, including timely Clinical Management of Rape (CMR) referral.

●         Provide accompanied referral support where a GDP beneficiary risks discrimination or refusal of service externally, and report such incidents to the Centre Manager.

●         Follow up referral outcomes and record them to ensure continuity of care and prevent loss to follow-up.

5. Clinical Setup, Commodities and Stock Management

●         Ensure functional clinical setup — examination bed, spotlight, privacy screen, lighting and running water — and maintain IPC equipment (autoclave, incinerator) and clinical equipment.

●         Maintain adequate, properly stored medicine stock; update registers, monitor minimum levels and expiry dates and prepare timely requisitions.

●         Escalate any actual or anticipated stock-out of essential commodities — including condoms, lubricants, HTC reagents and STI drugs — immediately to the Centre Manager and Quality Assurance Officer so replenishment occurs before services are interrupted.

●         Store DOTs blister packs safely until verified by PR/SR representatives and maintain incineration records.

●         Ensure condoms and lubricants are consistently available and discreetly accessible.

6. Documentation, Data and Reporting

●         Register clinic attendees accurately and maintain clinical records securely, applying coded identifiers where required; records containing HIV status, gender identity or GBV disclosure must be stored under lock with restricted access.

●         Submit monthly clinical and HTC reports to the Centre Manager and SR focal person within deadlines.

●         Implement clinical monitoring tools for STI, general health, TB and HTC services, review performance and report to the Quality Assurance Officer.

7. Community Engagement and Capacity Support

●         Support awareness sessions with GDP community members, peer educators, volunteers and frontline workers on HIV, STI, safer sex and available services.

●         Provide on-the-job technical guidance to outreach volunteers on health issue identification and referral.

●         Support sensitization of external health facility staff on GDP-inclusive, non-discriminatory service delivery.

8. Ethics, Confidentiality and Safeguarding

●         Uphold confidentiality, informed consent, non-discrimination and respect for dignity regardless of gender identity, expression or occupation.

●         Never disclose a beneficiary's HIV status, gender identity or attendance to family, community members or authorities without explicit informed consent.

●         Apply “do no harm” in all clinical and referral decisions, considering safety implications for the beneficiary.

●         Comply with Bandhu's Safeguarding, PSEA and Code of Conduct policies and report concerns through SHEA procedures.
