*Schedule and coordinate client care services based on client needs and caregiver availability.
*Conduct assessments and develop care plans in collaboration with healthcare professionals.
*Monitor and evaluate client progress and adjust care plans as needed.
*Ensure effective communication with all relevant departments for cases.
*Other duties will be assigned or as necessary
