Responsibilities & Context:

∎ *Schedule and coordinate client care services based on client needs and caregiver availability.
∎ *Conduct assessments and develop care plans in collaboration with healthcare professionals.
∎ *Monitor and evaluate client progress and adjust care plans as needed.
∎ *Ensure effective communication with all relevant departments for cases.*Other duties will be assigned or as necessary.
