Responsibilities & Context:
∎ Supervises and provides direct support to Community Outreach Volunteers (COV).
∎ Provide or Organizes training's for the Community Outreach Volunteers (COV) and traditional birth attendant (TBA) to ensure good technical skills for project implementation.
∎ Ensures good implementation of the Child protection, Health & WASH activities according to the donor agreement.
∎ Monitor, collect and report to the Community Outreach Officer any information linked to the living conditions and the health situation of the population (e.g. food security, vaccination status, nutritional situation, detection of medical needs of the population, number of people arriving or leaving the community, births and deaths in the community, vaccination status of children, etc.).
∎ Ensures good implementation of community outreach work, including health and hygiene education and disease surveillance according to the Outreach SOP.
∎ Responsible for accurate data collection of the community outreach work and nutrition activities according to the guidance provided by the line manager/programme manager.
∎ Performs any other duties requested by the Health Facility Manager according to his/her capacity and skills.
∎ Carefully follows up on EPI defaulters and ensuring COVs visit the household to encourage continuation in the vaccine and collects information on reasons for the default.
∎ Collaborates with the Health Facility Manager/Supervisor and others partners for ensuring beneficiaries health, CP & WASH facilities & need to ensure referral cases are accompanied to the correct referral facility.
∎ Maintain confidentiality regarding all information observed or registered.
∎ Support community-based mechanisms
∎ Map existing community structures and identify community mechanisms for preventing and responding to child protection, health and WASH issues.
∎ Identify vulnerable groups in the community as well as positive role model and engage with Support existing structures though community outreach and mobilization.
∎ Support community-led advocacy and access to services
∎ Link the community to service providers and local government stakeholders.
∎ Support the community to lead in developing local service mapping and referral pathway, including feedback on quality of services.
∎ Ensures timely implementation of community outreach work including health & hygiene education and disease surveillance activities
∎ Regularly accompanies the COVs & TBAs to the field to provide support and supervision to ensure professional and accurate education sessions are provided.
∎ Ensures COVs are correctly screening and referring patients from the community to the health facility according to established criteria.
∎ Ensures follow-up and support of ANC and PNC cases through community outreach, encouraging PLWs to visit the health facility for regular check-ups.
∎ Ensures correct data collection by the COVs and TBAs, checking & entry all data for completeness and accuracy before submitting the data to the KOBO toll and line manager.
∎ Makes recommendations to the Programme manager/outreach officer for any improvements needed for data collection or the template formats.
