Charge Nurse- Dialysis/ IPD Floor

• Diploma in Nursing Science & Midwifery.

• Valid BNMC registration is mandatory.

• Specialized training in Hemodialysis / Dialysis Nursing will be preferred.

• Supervise and ensure the assessment and reassessment of all patients on admission/ at beginning of the shift and documentation compliance with the Nursing documentation standard. Ensures safety and confidentiality of all records and patient information.

• Ensure safe and quality Nursing care for all patients in their specific area / unit in accordance to hospital protocols.

• Ensures Patient satisfaction and safe delivery of patient care. Safeguard patients’ safety at all times. Ensure work place and patient care areas always neat and clean.

• Provides direct patient care as and when required and in case of emergency situations, coordinating the duties of other staff to ensure appropriate action is taken in a timely manner.

• Attend consultant’s rounds and ensure instructions are followed by the staffs timely and appropriately. Ensure information from the doctors and different disciplines are recorded and acted upon as requested.

• Ensure all tests and investigations requested for patients are arranged on time as requested and appropriate preparation has been provided.

• Supervise and ensure all admission/ transfer/ discharge of patients smoothly and timely manner.

• Supervise and ensure safe and accurate medication management as per hospital policy

• Have full understanding of infection control measures and supervise the staffs ensure strict adherence to infection control policies by implementing universal precautions at all times and hand hygiene for all five moments.

• Supervise proper hand over of patient care information between different shifts of staff.

• Supervise and ensure the safe keeping / handover of patient belongings and valuables as and when required.

• Escalate any change in patient condition to concerned doctor / Consultant and senior nursing colleague/ Director Nursing without any delay

• Take bedside rounds regularly to ensure quality care is giving and ascertain need for additional or modified services within the department setting.

• Develop objectives and department policies for the management of the unit and submit to Nursing Administration.

• Prepare nurses’ duty roster and keep records of staff annual leave, medical leave, public holidays etc. Maintain staff discipline

• Delegate and designate nursing duties to ensure all patient care is carried out in a timely and effective manner.

• Utilize all personnel available to obtain maximum patient-care services as per defined N: P ratio.

• Operate the unit within budgeted levels for staffing, supplies and equipment – recommend staffing patterns and standard of Nursing care to Nursing Administration.

• Co-ordinate with personnel of other departments and nursing personnel in providing quality and safe patient care.

• Evaluate and appraise work performed by all nursing staff members and provide staff with information necessary for improving the performance of their jobs.

• Identify and investigate nursing problems, grievances from staff, patients and visitors and assist to resolve them.

• Assist to conduct the orientation programs, Continuing Nursing Educational programs and clinical nurse trainings.

• Conduct regular staff meetings and submit minutes of these meeting to HOD.

• Conduct active and closed medical record audit regularly.

• Report and document all adverse events / incidents related to patients, visitors, staff and hospital properties promptly.

• Supervise and ensure judicious utilization of supplies to avoid wastage. Ensure availability of adequate supplies and equipments in good working condition at all times for smooth functioning of the unit and non-functioning equipments are dispatched for repair if necessary.

• Ensure billing of all bedside procedures and devices accurately.

• Conduct monthly inventory audit and report to Director Nursing.

• Arrange for ambulance service as and when required.

• Has sound knowledge and skills on handling Biomedical equipments like Crash cart, Defibrillator, Syringe Pump, Infusion Pump, Air Mattress, Glucometer, Nebulizer Machine, B.P Apparatus, Suction Apparatus, Pulse Oximeter, ECG Machine, BIPAP/CPAP, Hair Trimmer, Oxygen Shut Valve, and Wheel Chair/Stretcher etc. Train the staffs on the same and supervise them handling the equipment.

• Maintains professional etiquettes, timely escalation and problem solving.

• Attend and ensure maximum participation of staffs in CNE conducted by the department and in all mandatory training as per HR policy

• Participate in hospital safety codes as per given responsibility and situation

• Ensure strict adherence to the hospital policies and nursing code of conduct by all staffs.

Perform any other task assigned by the department.

Area Specific job description

• Ensuring all bed side procedures are safely performed by nursing staff following the aseptic technique.

• Renal Biopsy, insertion of HD Catheters in Jugular, Femoral and Subclavian routes.

• Ensure proper placement of vascular access and/or catheter

• Ensures appropriate AV Fistula care and needling

• Guiding staff in performing Dialysis cycle and procedure.

• Ensure disinfection of the equipment after each dialysis

• Ensure safe nursing care to acute and chronic hemodialysis and end-stage renal disease (ESRD)
