Job Responsibilities:
∎ General Job Description:
∎ Setting up research and educational activities Liaises with external agencies / Organizations, as required in conducting and organizing seminars.
∎ Attends and contributes to interdisciplinary team meeting relating to clinical investigation drug trials.
∎ Performs education & Training responsibilities including orientation and induction and performance evaluation.
∎ Provides drug information to physicians, nurses and other health care professionals, as related to doses drug interactions and other related issues and in clinical investigational trial drugs.
∎ Training the stuff within the pharmacy and physicians and nurses on medication management policies.
∎ Oversees medication errors drug interactions and adverse drug events reporting. Assisting in the root cause analysis of the above mentioned. Recommending / Advising on selection of appropriate drugs and medicines in consultants. (Consultation Service)
∎ Oversee performance improvement and accreditation related activities, and statistical analysis and interpretation of related departmental quality indicators.
∎ Departmental Specific Job description:
∎ To check and input the patient information of weight, height, body surface area, body mass index.
∎ To input the allergy information such as Drug Allergy, Food Allergy and Other Allergies.
∎ Medication Chart evaluation (Prescription Screening)
∎ Check the appropriateness of the medication ordered.
∎ Check the appropriateness of the dosage form ordered.
∎ Check the appropriateness of the dose ordered.
∎ Check the appropriateness of the Frequency ordered.
∎ Check the appropriateness of the route ordered.
∎ Monitor rational use of restricted antibiotics as well as check patient?s Culture Sensitivity Test, Serum Creatinine, Creatinine clearance.
∎ Report PRD department if more than 3(three) antibiotics use concurrently
∎ Monitor rational and safe use of High Alert (Concentrated Electrolytes) and High Risk (IV Narcotics, IV anticoagulants, Insulin and Chemotherapeutic medicines.
∎ Monitor the Antibiotic Prophylaxis prior to surgery as well as Skin Sensitivity Test.
∎ Screening for possible duplication of therapy
∎ Screening for possible interaction with other medications
∎ Screening for possible allergies or sensitivities
∎ Screening for other contraindications.
∎ Service Management:
∎ Provide information to Doctors, Nurses, and customers queries about medicine.
∎ Taking suggestion from doctors to improve qualities of medicine service and report to the unit supervisor.
∎ Perform all other tasks as required by the department
∎ Communicating effectively with staffs at all levels and patients and ensure support service
∎ Address any operational issues that require intervention / coordination
∎ Coordinate technical assistance to patient in terms of basic medical knowledge and application.
∎ Training & Development:
∎ To participate in the academic activities of the department and hospital by taking part in:
∎ Mandatory Safety Training
∎ Prevention & Control of Infection
∎ Departmental Policy & Procedure
∎ Trained in Basic CPR
∎ Trained on blood borne pathogen exposure & Management.
∎ Additional Job Responsibilities
∎ Prevention and control of infection:
∎ He / She shall follow the Standard Precautions for all patients as per hospital protocol for prevention and control of infections. Standard precaution includes but not limited to following:
∎ Hand Hygiene
∎ Respiratory Hygiene
∎ Proper use of personal protective equipment (PPE)
∎ Aseptic precaution
∎ Prevention of accidental inoculation injury, and
∎ Waste segregation
∎ He / She shall follow the Additional Precautions for the suspected / confirmed patients of certain infectious diseases as per hospital protocol for prevention and control of infection
∎ He / She is responsible for implementing and maintaining other practices and protocols of PCI as per hospital PCI Policy.

∎ Age 25 to 30 years
