∎ The applicants should have experience in the following area(s):
health program hospital clinic
Job Responsibilities:
∎ Visiting care receiver house to assess health condition
∎ Create medical reports
∎ Provide training to healthcare professionals
∎ monitor weekly reports
∎ Follow-up with healthcare professionals for updates and support if there is any medical requirement
∎ measure performances and provide feedback to improve service
∎ provide training to increase quality of healthcare service providers
∎ Working with partner organizations to develop customized service
∎ Appropriate evidence based screening, risk assessment and early detection of illness, disease and disability.
∎ Interventions to reduce or change risky and harmful lifestyle behaviour.
∎ Ongoing care and support for care receivers with chronic and terminal conditions to reduce deterioration, increase independence and reduce suffering linking, where relevant, with appropriate service providers.
∎ Health education, counselling and information provision about how to improve health
∎ Ensure all care receivers services undertaken are charged out in accordance with appropriate protocols.
∎ All staff are informed, as appropriate, about anything that they should be aware of regarding care receivers, or regarding the internal workings of the practice.
∎ Maintain liaison with other healthcare professionals
Additional Requirements:
∎ English proficiency in writing and speaking: Professional
∎ Understanding of elder care, homecare and community care
∎ MS Office skills, specially excel
