Responsibilities & Context:
Assist with activities of daily living (ADLs).
Help with transfers (e.g., bed to wheelchair), walking, and light exercise to maintain physical mobility and prevent falls.
Remind clients/the patient to take prescribed medications on time. Observe and report any changes in physical or mental health to family members.
Provide emotional support, engaging conversation, and companionship. Accompany the patient to medical appointments, errands, or social outings.
Maintain accurate daily logs of care provided, activities, and any notable incidents or health changes.
Take review from the patient about treatment and services which can be a video or voice record.
Others task assigned from the authority.
