Educational Requirements:
∎ Minimum Master's degree in Pharmacy / Clinical Pharmacy/Pharmacology from any reputed institute of Bangladesh
∎ Preferred Professional Certification: Must have valid Pharmacy Council Registration

Job Responsibilities:
∎ Develops, enforces and maintains policies and procedures that promote and enhance patient outcomes through safe, effective, cost-efficient and appropriate medication therapy use for patients receiving antimicrobials.
∎ Coordinates programs designed to minimize medication errors, adverse drug reactions and medication misuse with regards to medication preparations in infectious disease through improved reporting, analysis and follow-up.
∎ Coordinates and maintains the antimicrobial stewardship program, including:
∎ Assisting in development and maintenance of an antimicrobial formulary approved by the relevant auhority.
∎ Promoting optimal use of antimicrobial agents for prophylactic, empirical, and definitive therapy.
∎ Aiding in appropriate selection, optimal dosing, rapid initiation, proper monitoring and de-escalation of antimicrobial therapies.
∎ Developing restricted antimicrobial-use procedures, therapeutic interchange, treatment guidelines, and clinical care plans.
∎ Working with the Antimicrobial Stewardship committee to maintain and update the Hospital Empiric Therapy Guide.
∎ Overseeing daily prospective audit of antimicrobials.
∎ Monitoring and evaluating antimicrobial use in prophylactic, empirical, and definitive antimicrobial therapy and intervening if required.
∎ Generating and analyzing quantitative data on antimicrobial drug use to perform clinical and economic outcome analyses.
∎ Reporting data and findings to the Antimicrobial Stewardship, Infection Control and Pharmacy & Therapeutics Committees.
∎ Coordinating, facilitating and promoting education, training, competency assessment and performance improvement of pharmacists, and other healthcare professionals in the appropriate use of antimicrobials
∎ Serves as pharmacy consultant for any antimicrobial use.
∎ Works with infection control team in developing and maintaining strategies aimed to minimize the risk of the emergence of resistant strains of microorganisms
∎ Works with the microbiology laboratory personnel to ensure that appropriate microbial susceptibility tests are reported on individual patients in a timely manner, and collaborating with the laboratory, infectious diseases specialists, and infection preventionists in compiling susceptibility reports (at least annually) for distribution to prescribers within the hospital to guide empirical therapy.
