Job Responsibilities:
∎ 1. Inventory Management of Medicine:
∎ Ensure the DGDA, Narcotic and BSTI compliances
∎ Development of Hospital Formulary with communicating with Consultants and PTC
∎ Monitoring the out of stock and stock holding days to ensure availability of all drugs in respective stores and specialists’ area.
∎ Ensuring product receiving, indent and issue and GRN and Bill settlement to supply chain.
∎ Ensuring safety storage and Expiry Monitoring and RTS processing
∎ 2. Prescription Screening and Monitoring: Review each prescription with all relevant patient information to handle and individualize drug therapy according to standards
∎ 3. Medication Service:
∎ Prepares and/or oversees the preparation and dispensing of prescription medications to patients or medical practitioners; makes decisions regarding generic substitution of prescribed pharmaceuticals.
∎ Being responsible for the accurate dispensing and timely distribution of drugs.
∎ Counsels the patients and/or medical practitioners on drug indications/contraindications, dosage, drug interactions, and side effects; provides other drug and pharmaceutical information
∎ 4 Medication Error & ADR Reporting: Identify the medication error by checking Prescription and Medicine Distribution , ADR monitoring and reporting , identify trends to prevent future ADR, define the role of medical, nursing, pharmacy and other healthcare personnel in ADR reporting, to ensure
∎ Responsibilities/Tasks:

∎ 6.Chemotherapy Medicines: Calculation of medicine’s dose and proper selection of a solvent/diluents and preventing medication errors by reviewing the prescription and educating nursing professionals on safe handling and administration of Cytotoxic Drugs.
∎ 7. Training: Provides guidance and training to pharmacy staff and monitors their performance.
∎ Involves in the training and education of medical, pharmacy and nursing personnel in the patient care areas.
∎ 8. Provide Drug distribution support and pharmaceutical care to transplant Patients.
