Responsibilities & Context:
∎ Receive, review, and process patient medical records for completeness and accuracy.
∎ Organize and file medical records, reports, and correspondence in a timely manner.
∎ Maintain both electronic health records (EHR) and physical records, ensuring data integrity, at Hospital soft wear.
∎ Scan, index, and upload documents into the EHR system.
∎ Retrieve and deliver medical records upon request from authorized personnel.
∎ Ensure proper storage and security of all medical records.
∎ Accurately enter patient demographic and medical information into the EHR system.
∎ Update patient records with new information, test results, and physician notes.Verify and correct data discrepancies.
∎ Maintain accurate logs and tracking systems for record movement.
∎ Process requests for release of medical information in accordance with hospital policies.
∎ Verify the validity of release requests and ensure proper authorization.
∎ Prepare and distribute copies of medical records to authorized parties.
∎ Maintain logs of all released information.
∎ Adhere to all hospital policies and procedures regarding patient confidentiality and privacy.
∎ Comply with health ministry regulations and other relevant legal requirements.
∎ Maintain the security of medical records and prevent unauthorized access.
∎ Participate in audits and quality assurance activities to assistant only.
∎ Provide excellent customer service to patients, physicians, and other healthcare professionals.
∎ Respond to inquiries and requests for information in a professional and timely manner.
∎ Assist patients with completing medical record requests.
∎ Assist with the destruction of outdated medical records according to hospital policy.
∎ Maintain and order supplies for the medical records department.
∎ Assist with special projects as assigned.
∎ Participate in training and continuing education to maintain proficiency in medical records management.
