∎ Age at least 35 years
Responsibilities & Context:
∎ Provide technical support to Medical Assistants and Peer Navigators and coordinate for case management services with them
∎ Maintain close interaction and communication with all PLHIV population.
∎ Regular coordination with DIC/Sub DIC/Outlets and TB service-oriented service centers for TB screening and treatment center as well as coordination with ART center (Peer Counselor, Doctor, Lab technologist etc.) for services
∎ Provide motivational and adherence counselling to PLHIV member for HTS of PLHIV’s regular partner
∎ Support to organize quarterly social day and communicate with PLHIV member
∎ Conduct Social Day-provide requisition and adjust the fund.
∎ Maintaining communication with the clients who are living with HIV (PLHIV)Support to provide the treatment those who are taking regular basis of ART
∎ Regular home visit of PLHIV population as and when required and ensure the adherences to the treatment.
∎ Support to Peer Navigator to refer ART center and regular visit ART center for tracking ART adherence based on context.
∎ Support to Peer Navigator for admission to the hospital emergency basis if any PLHIV member become sick
∎ Support PLHIV members for HIV discloser to partner, family and to any others if required.
∎ Prepare the monthly and quarterly progress report of PLHIV population
∎ Prepare field visit report, social day report and home visit report and share with line manager
∎ Maintain audio and visual confidentiality of client information in each and every aspect
∎ Support and act for keeping privacy and confidentiality in relation to PLHIV care and services each and every aspect discuss with Bandhu HR.
∎ Drive and ensure 100% /ART enrollment target in favor of detected PLHIV
∎ Visit the project implementing areas/ sites
∎ Any other assignment as and when required by the management/concerned authority.
